• Contracting health providers & vendors: In addition to providing front-line services, VHA will subcontract with various partner SPOs and equipment, supply and pharmacy vendors to support a program in a given geography.
• Navigating Care: Connecting clients to various care services across the continuum to meet their needs.
• Coordinating & Scheduling: Backoffice team supports management of client records, referral assignments and home care coordination.

• Program Management: Providing resources to support the development, implementation and growth of the program utilizing co-design values.
• Contract Management: Development of contracts to ensure program and legal requirements are met, compliance processes confirmed, and clear understanding of parties’ responsibilities established. This also includes ongoing contract management and enforcement.
• Quality & Best Practice & Design Support: Identify/support best practices and provide recommendations or develop standards, policies, and programs to address gaps, needs, and develop and deliver education as required and models of care as needed.

Digital Health Record Cloud-based clinical information systems that support the delivery of client care via client scheduling, clinical documentation, providing families access to relevant information, and facilitate communications with the care team. Reporting utilizing our data warehouse to provide data visualization to monitor and support improvements in service delivery. Collaborative Workspace to allow real-time communication between partners.
