As one of the largest not-for-profit home care providers in Ontario, VHA Home HealthCare (VHA) has strong connections in the communities we serve and is known for its collaborative partnership and high quality, client-centred care. VHA is an active member in multiple Ontario Health Teams in the regions where we provide service and also actively participates in other integrated care initiatives with partner organizations. VHA invests in research and innovation and in integrated care solutions to help make care better, for the clients and families we serve, for our providers and for the sector.
In every role and at every level, we lead with purpose, build bonds that matter, and we’re passionate about providing spectacular care. VHA is accredited by Accreditation Canada with exemplary standing, a Registered Nurses’ Association of Ontario Best Practice Spotlight Organization (BPSO) and an anchor partner of the United Way Greater Toronto.
Lead Home Care Agency
The Lead Home Care Agency (LHA) model is a new model of care that provides a more seamless experience for patients, caregivers and health care providers to improve patient outcomes and population health while also being more cost-effective. The LHA model provides enhanced and more integrated supports throughout a patient’s care journey, including the transition from hospital to home.
Transitional Care Programs
Acting as a pressure-relief mechanism, VHA works with hospitals to safely transition individuals receiving care in inpatient hallway beds or those waiting to be admitted to the ED, providing care for them in transitional spaces. The Transitional Care Program (TCP) serves as a transitional space for clients to receive the level of care they require, which includes personal support, nursing, physical therapy, occupational therapy, in a community setting prior to discharge to their destination: home, long-term care, assisted living or hospice.
Healthcare Response Unit
In response to the changing needs of Ontario’s health care system, in 2021, VHA created THRU, which stands for Tactical HealthCare Response Unit. THRU is a strategic team of experienced nurses, pharmacists, pharmacy assistants and non-clinical staff who can urgently respond to community health needs and challenges anywhere in the Greater Toronto Area.
Research and Innovation
VHA is dedicated to supporting research and innovation that advances human welfare, creates new knowledge, develops our understanding and informs best practices in home and community care. Our research focuses on key areas of home and community care that create possibilities for more independence, support clients to live at home, promote safe and rewarding working conditions for home care providers, and build toward fully integrated care across sectors. We strive to break new ground in research and development of integrated solutions that reflect the voices of our clients, caregivers and providers.
