Senior Friendly 7

What's the challenge?

The Regional Geriatric Program of Toronto, the leading authority in the care of frail seniors, has identified the ‘Senior Friendly 7 (SF7)’ clinical areas of practice (delirium, mobility, nutrition, polypharmacy, pain, continence, and social engagement) for which routine monitoring and interventions are most likely to improve health and quality of life of frail seniors or those at risk of frailty. The goal of this initiative was to develop SF7 tools for Home and Community Care.

What did we do?

As PSWs and their coordinators are key providers of Home and Community Care, this project evaluated the SF7 resources to ensure that this toolkit will be useful to PSWs and their coordinators.PSWs who provide care to frail older adults evaluated the toolkit through a series of 3 workshops. Their feedback focused on ensuring that the content developed in the toolkit was relevant and that it provided the right resources to support their work.
