Adams House

In 2002, Adams House, a supportive housing complex located in downtown Toronto, opened its doors to help people living with a wide range of mental health problems realize a better life and future by providing a safe and clean alternative to life on the streets, shelters and rooming houses.

The home, which houses 27 tenants, is a joint venture between VHA and COTA Health.

House Calls

This exciting  partner project between VHA Home HealthCare, COTA Health and SPRINT, under the direction of Dr. Mark Nowaczynski, began in 2007 to help frail seniors in north western Toronto access primary health care. These seniors may:

  • Have trouble leaving their home to attend medical appointments
  • Be suffering from a cognitive impairment or mental illness and need help managing care, medication, daily activities, etc.
  • Need ongoing care for a chronic condition that, if left untreated, will result in institutionalization or unnecessary or prolonged hospital stays

In the summer of 2009, new Aging at Home funding was approved by Toronto Central Local Health Integration Network (LHIN) to support an expanded House Calls team. Nursing Support

VHA is partnering with various community support service agencies in the Central and Toronto Central LHIN communities to provide a range of nursing assistance to tenants in supportive housing buildings as part of the Aging at Home initiative.
