Adams House
In 2002, Adams House, a supportive housing complex located in downtown Toronto, opened its doors to help people living with a wide range of mental health problems realize a better life and future by providing a safe and clean alternative to life on the streets, shelters and rooming houses.

The home, which houses 27 tenants, is a joint venture between VHA and COTA Health.

House Calls
House Calls is a not-for-profit program providing frail, vulnerable, marginalized and homebound seniors with physician-led interdisciplinary care at home. 

Eligible Clients

  • Are 65 years or older;
  • Live in the House Calls catchment area (see map, below):
  • Have great difficulty accessing a family physician because of physical, cognitive and / or psychiatric disabilities.

The House Calls Team
Under the leadership of primary care physician Dr. Mark Nowaczynski, the integrated House Calls team includes a Social Worker, Occupational Therapist, and Nurse Practitioner, supported by a team coordinator.

House Calls service is available Monday to Friday, 8:30am to 4:30pm.

 


No Cost to Clients
House Calls is supported by the project partners and the Toronto Central LHIN, through Ontario’s ‘Aging at Home’ strategy

Nursing Support
VHA is partnering with various community support service agencies in the Central and Toronto Central LHIN communities to provide a range of nursing assistance to tenants in supportive housing buildings as part of the Aging at Home initiative.
