Community Support Programs

As a not-for-profit, charitable organization, VHA is committed to serving the most vulnerable in the community. VHA delivers several programs through its own charitable resources and with the support of funding partners—including United Way, the City of Toronto and Central Local Health Integration Network (LHIN).

For more information on any of these programs and an eligibility assessment, please contact our Intake Social Worker at 416‐489‐2500 ext. 4339 or 4192.

  • Attend support groups/counselling
  • Attend parenting classes
  • Go to their own medical appointments
  • Get needed rest and sleep
  • Connect to their own community resources

Caregiver Relief – Relief for live-in caregivers to reconnect and recharge from caring for a chronically ill family member.

Homemaking and Personal Care – Services help maintain a healthy and safe home environment after a recent hospital discharge or when recuperating from surgery or injury.

Child and Family – Child care support for families in recovery after a hospital discharge or a medical crisis to ensure the least amount of disruption to children and families.

Teaching and Mentoring – One‐on‐one support for first-time parents, who need mentoring on child care, establishing routines and household management.
