We are currently seeking a:
Clinical Lead– Physiotherapy Services (Toronto Central)
To provide physiotherapy consultation, assessment and planned interventions for clients through VHA Rehab Solutions.
Responsibilities:
· To provide physiotherapy consultation, assessment and planned interventions for clients in the Toronto region through VHA Rehab Solutions.
· Assessment and implementation of physiotherapy interventions.
· Assess clients using interviews, standardized and informal assessment techniques in accordance with professional practice guidelines and standards established by VHA Rehab Solutions.
· Attend client related meetings to report on assessment findings and discuss plans for interventions
· Progress reports and discharges as required.
· Document all assessment findings, treatment plans, progress and discharge summaries of interventions as required by the profession, referral source and the agency.
· Implement client interventions based in assessment information, client input and in conjunction with the client's integrated care plan.
· Ensure that services provided are best practice, evidence based and evaluated through the routine use of valid and reliable outcome measurement.
· Adheres to the code of ethics of the profession, in maintaining standards of practice for the profession as defined by the Ontario College of Physiotherapists.
Clinical Lead Role:
Among other roles, you will provide advice, consultation and expertise related to the Occupational Therapy profession. You will provide joint visits with Service Providers to give input for performance reviews and to support Service Providers who face clinically challenging situations. You will provide clinical orientation for new Service Providers or those changing clinical areas, and mentor groups for new grads or those new to the community setting. You will identify, promote, and participate in best practice initiatives reflective of the requirements of the evolving health care system. You will also meet regularly with Regional Managers. You will act as a resource in the implementation and interpretation of discipline-specific professional standards of practice, contribute to the development of standards of practice, clinical policies and procedures relevant to the profession.
Education and Experience:
· Baccalaureate Degree in Physiotherapy from a recognized school of physiotherapy.
· Registered with The College of Physiotherapists of Ontario.
· Minimum of 2 years clinical experience, preferably in the community.
· Specialized courses to support areas of clinical expertise.
