Our caring team of professionals provide nursing, rehabilitation, personal care and home support services to people who need help at home, school, work or in a long-term care facility, 24 hours a day, seven days a week.

London – 1 Full-time and 1 Part-time - Adult Population

To provide Occupational Therapy consultation, assessment and planned interventions for clients in the London area.

The Occupational Therapists works within a continuous quality improvement framework, which reflects the VHA vision and mission of services as well as promoting and maintaining good public relations with clients, customers, VHA staff, volunteers and associates.
Responsibilities (but not limited to):

• Assessment and implementation of Occupational Therapy interventions in a community setting.
• Assess clients using interviews, standardized and informal assessment techniques in accordance with professional practice guidelines and standards established by VHA Rehab Solutions Health.
• Attend client related meetings to report on assessment findings and discuss plans for interventions, progress reports and discharge as required.
• Ensure coordinated delivery of care through ongoing communication with all members of the client's team regarding plans and schedules for interventions.
• Document all assessment findings, treatment plans, progress and discharge summaries of interventions as required by the profession, referral source and the agency.
• Implement client interventions based in assessment information, client input and in conjunction with the client's integrated care plan.
• Ensure that services provided are best practice, evidence based and evaluated through the routine use of valid and reliable outcome measurement.
• Adheres to the code of ethics of the profession, in maintaining standards of practice for the profession as defined by the College of Occupational Therapists of Ontario.
• Designs and adapts special devices such as splints, canes, walking aids, etc.
• Prepares statistical records of client care and related activities as required by VHA Rehab Solutions Health, the Ministry of Health and other agencies.

Knowledge and Skills:

  • Clinical Expertise to provide a variety of Occupational Therapy interventions.
  • Knowledge of community care requirements and resources available.
  • Good organizational and planning skills.
  • Excellent consultation and teaching skills.
  • Utilizes an evidence based clinical practice.
  • Familiar with routine use of valid and reliable outcome measures to support clinical interventions.

 

Other:

  • Observes VHA Rehab Solutions rules and regulations, policies and procedures, practices and current legislation, in particular the Human Rights Code.
  • Complies with all Health and Safety policies and procedures to ensure every reasonable precaution is taken for the protection of workers and clients and that health and safety concerns or incidents are responded to promptly.
  • Works cooperatively and constructively with all members of staff.
  • Ensures confidentiality of client, employee and VHA Rehab Solutions information is maintained at all times.
  • Undertakes other related and/or delegated duties.

Education and Experience:

• Registered with the College of Occupational Therapists of Ontario.
• Baccalaureate Degree in Occupational Therapy from a recognized school
• Ability to work independently.
• Specialized courses to support areas of clinical expertise.
• Knowledge of community care requirements and resources available.
