Client Service Coordinator – Extreme Cleaning Program
3 days per week (21 hours/week) - Contract position approximately 7 months (end date March 31, 2011)
Incumbent must have a driver’s license and a vehicle, preferably a van to provide delivery of supplies to various work sites.
Reporting to the Manager, Community Support Programs you will conduct intake and assessments for the “Central LHIN: Extreme Cleaning Enhancement Project”. Develop and implement care plans based on the assessment at the intake level. Provide information and referrals to the community. Receive and process all requests for service from the referral source, support service development and administrative activities. Provide administrative, planning and scheduling support to the Community Support Team. Participates in service and quality initiatives. Working as a member of the Community Support Team, you ensure compliance with VHA’s standards of excellence.
Degree in Social Work (BSW) or Social Service Worker diploma combined with 2-3 years of related experience in community health care and social services with sound knowledge of assessments, counseling and case coordination. Excellent knowledge of community and health related programs and resources in North York and York Region.
You are an effective communicator, with a pleasant telephone manner in all of your interactions with clients, staff and funders. You are culturally competent, interacting effectively with diverse stakeholders with a wide range of communication skills, knowledge and understanding of the home care system. You are a keen problem solver with an eye for detail and the ability work on a micro and macro level at the same time. You possess strong computer skills with proven speed and accuracy and have experience learning and utilizing new software applications; working knowledge of Gold Care is a definite asset. Ideally you have coordinated community support services (personal support/nursing/homemaking) and have a comprehensive understanding of medical terminology and of the range of services provided.
You are a strong team player and can work effectively under pressure, adhering to deadlines. You are comfortable working in a constantly shifting environment as VHA responds to changes in client needs and staff availability. You have the ability to integrate many different pieces of information to produce a cohesive plan. Knowledge of the City of Toronto, with emphasis on North York and York Region and TTC routes will assist you in ensuring VHA’s valuable staff resources are deployed effectively. You possess relevant post secondary school education/training combined with practical experience. Proven organizational and self management skills are required. Demonstrated efforts to continuously develop and upgrade skills and achieve personal and organizational performance targets are required. Knowledge of a second language, preferably Mandarin, Cantonese or any South Asian language, is a definite asset.
Salary range: Level 6 - $26.37 per hour
